The New York State Department of Education requires an annual physical examination for new entrants, and students in Grades K, 2, 4, 7 and 10. For students who do not have a private physician, HealthWorks-WNY can provide these physician services at your school in the fall and spring. New York State Education Law states that the physicals must be performed by a New York State Physician. Without a physical exam and the necessary paperwork, students cannot attend school.
Children in various grades participate in sports or physical education sponsored by the schools. For students who do not have a private physician, HealthWorks-WNY can provide the physical assessment to appropriately approve participation in such activities. These medical services can be conveniently performed at your school.
All full time undergraduates, including transfer students, and all international graduate and undergraduate students, are required to submit a completed physical examination and medical history form prior to their first class attendance. Physical examinations must be completed by March 1st for current year Fall attendance and after August 1st for the following year Spring attendance.
